Health chiefs have joined forces with the voluntary sector to help frail and elderly people live independently in isolated areas.

The NHS North Derbyshire Clinical Commissioning Group, which represents 37 GP practices and acts on behalf of over 288,000 patients, has contracted the Dales Council for Voluntary Services to work with organisations to carry out basic tasks for the people at most risk of hospital admission without practical support at home.

Services to be provided include collecting and delivering prescriptions.
