The social workers who failed to act when Hannah Bonser was badly neglected as a child will not face the consequences, it has emerged.
None of those who dealt with her case in the late 1990s still works for Doncaster Council social services.
Director of Children’s Services at the authority, Chris Pratt, said: “Those staff have all moved on.”
He said had the case been more recent, there may have been disciplinary proceedings.
Mr Pratt said awareness of child cases involving neglect had since been raised to the extent that six years ago there were 75 such cases in Doncaster but now there are 395.
“That is a sign that a child being neglected is treated more seriously because we know it can cause major damage to a child,” he said.
“Reports like this always show how we need to improve in future.
“Today, we are much more aware and experienced with cases like this and we are working with many more children in terms of identifying and supporting their needs around neglect.”
Christine Bain, chief executive of RDaSH, which provides local health services, said: “We accept all the findings and recommendations made following this investigation and we acknowledge that our service to Miss Bonser should have been much better.
She added: “We have already taken action to improve our services to patients and service users and a detailed action plan is being implemented.
“The external investigation has not found a reason or cause as to why Miss Bonser carried out this attack and she had never, to our knowledge, previously harmed anyone.
“We will continue to review and improve our services for patients with a borderline personality disorder.”
