SOUTH Yorkshire residents are being invited to help shape the future of NHS treatments and services by completing a questionnaire.
The tick-box questionnaire, which takes between five and 10 minutes to complete, aims to improve the long-term health of people across the region and covers lifestyle, work and background as well as their health, long-standing conditions and experience of healthcare.
Replies can be returned to Sheffield University at no charge.
Alternatively log on to http://syc.shef.ac.u and completing the form online.
The document, accompanied by a covering letter from a GP, will land on doormats in Sheffield, Rotherham, Barnsley and Doncaster this week.
Researchers want to recruit around 20,000 local residents and are particularly keen to receive responses from men and young people.
All information provided is strictly confidential and answers are anonymous.
Anyone completing a questionnaire can withdraw their consent at any time.
Participants will be asked for permission for researchers to access their health records.
